Thyroid disorders are  linked to so many diseases.  They are often missed, and mistreated


Thyroid disorders can be a silent killer in some cases, and can cause unnecessary daily sufferings and miseries in many others.  Unfortunately it is the most unrecognized, under-treated disorders.

TSH is not a thyroid test, but is a pituitary test which may not be a diagnostic test and  it can miss many cases of hypothyroidism.  Even if TSH is high, it is not taken seriously by many doctors and patients are told their test is normal, even though patient may be symptomatic.  If treatment is started, only Synthroid (Levothyroixine) is considered, and the dose is adjusted based on the TSH reading not by patient's symptoms. Synthroid, which has T4 only may not cover all of the patient's symptoms.

Synthroid (levothyroxine ,Levothroid, Levoxyl) is synthetic product  and has only T4. Is T3 important?  Yes, indeed T3 is more important than T4, because it is free T3 which is more functional and better for cell operation. A study from New England Journal of Medicine has shown that adding T3 to T4 enhance mood and brain function. Another study showed T3 impact significant improvement in Chronic Fatigue Fibomyalgia patients. These studies and more shows T3 and possibly other parts of thyroid have important role to play and they should not be ignored.

How to solve this problem? Give T3 along with T4 or better yet give natural thyroid products like Armour Thyroid, Nature-Thyroid, Westhroid. They are commonly called Desiccated Thyroid Hormones and these hormones contain almost all of the products the human thyroid has and human body needs.

In my opinion, if  hypothyroidism is considered  along with TSH, patient's  fasting basal temperature and pulse  must be checked  and other tests like T3,T4 ,Free T3,Free T4 , Reverse T3, Thyroid antibodies ,Thyroid Ultrasound may be needed.

HOW T4 CONVERSION PROBLEMS CAN CAUSE HYPOTHYROIDISM, EVEN WHEN TSH IS NORMAL

Perhaps you are on thyroid medication and you are still symptomatic, but your TSH is “normal.” There can be many causes for this. The most important and most common cause is that the conversion of T4 to T3 is not occurring.

Remember that T4 is not functional itself, while free T3 is. Free T3 is the one that acts on every cell and makes every cell of our body functions properly. T4 has to be converted to free T3 for this to happen. However there are many situations where this conversion is interrupted. For example, excess hormones, especially estrogen, birth control pills, and the presence of heavy metals like mercury, lead, arsenic and fluoride, bromide etc. This interruption of the conversion to T3 is an indicator of hypothyroidism even when T4 and TSH are normal. It follows that patients receiving Synthroid (T4 only medication) for these problems suffer more.

Many physicians are taught to only look at TSH .They will tell their patients that their thyroid is normal, but in fact it is not. A thyroid deficiency needs to be detected and treated as early as possible. Hypothyroidism has been known to cause many secondary illnesses, some of them life-threatening like heart attacks and arrhythmia. Many are debilitating like dementia and mental retardation. Early detection of a thyroid deficiency, including T3 deficiency, is very crucial.

FREE T3 IS A BETTER TEST
 
TSH is a pituitary test and reflect thyroid function indirectly. When your thyroid is not working pituitary is stimulated and TSH is secreted by pituitary,  and vise versa. When too much thyroid is produced TSH secretion the pituitary is reduced.
 
But does it really show if the thyroid hormone is being used by your cell?
 
Some patient with normal TSH continues to have signs and symptoms of thyroid deficiency or excess
and their fasting temperature remains abnormal especially, those who have T4 conversion problems.
 
When we test free T3 on these patients , some have abnormal levels of T3 , although their TSH may be normal. Adjusting dose according to free
 T3 and  fasting temperature may improve signs and symptoms of these patients.
 
So just looking at TSH numbers and not listening, observing and examining patient and not considering other tests if patient still not well IS
JUST NOT A GOOD MEDICINE.

WHAT IS REVERSE T3 AND WHY IT IS IMPORTANT TO TREAT IT ? Dr. Tahir MD
In a healthy state,T4 is converted  to T3 which is the active
form that acts on every cell of body to provide necessary
energy. However, in certain conditions like estrogen dominant state,
infections, iron deficiency and adrenal abnormalities  etc.T4 may convert
to reverse T3. Reverse T3 will  compete with free T3 for its action on
the  cells making cells functionally hypothyroid.
In this scenario, although other lab results like TSH may look normal,
the patient is thyroid deficient and has signs and symptoms of hypothyroid. Many

STUDY SHOWS PATIENTS RECEIVING LEVOTHYROXINE (SYNTHROID)  AND NORMAL TSH  MAY STILL BE SYMPTOMATIC 

 
'Impaired psychological well-being, depression or anxiety are observed in
5-10% of hypothyroid patients receiving levothyroxine, despite normal TSH
levels. Such complaints might hypothetically be related to increased free T4 and
decreased free T3 serum concentrations, which result in the abnormally low free
T4:free T3 ratios observed in 30% of patients on levothyroxine. Evidence is
mounting that levothyroxine monotherapy cannot assure a euthyroid state in all
tissues simultaneously, and that normal serum TSH levels in patients receiving
levothyroxine reflect pituitary euthyroidism alone

A TRUE STORY
 
I have a patient who has had infertility issue for many years She saw some gynecologists and other specialist
and they did very expensive work up and care but got no results. She came to me
with complaints of fatigue. Some people including doctors  told her

Abstract
Recent evidence has suggested an association between sub-clinical
hypothyroidism (SCH) and microalbuminuria in patients with type 2 diabetes.
However, whether SCH is related to microalbuminuria among subjects with
prediabetes has not been studied. Thus, we evaluated the association between SCH
and microalbuminuria in a cohort of prediabetic Egyptian adults.
 
 METHODS:
 
A total of 147 prediabetic subjects and 150 healthy controls matched
for age and sex were enrolled in this study. Anthropometric measurements, plasma
glucose, lipid profile, homeostasis model assessment of insulin resistance
(HOMA-IR), thyroid stimulating hormone (TSH), free thyroxine, triiodothyronine
levels, and urinary albumin-creatinine ratio (UACR) were assessed.
 
RESULTS:
The prevalence of SCH and microalbuminuria in the prediabetic subjects
was higher than that in the healthy controls (16.3% vs. 4%, P<0.001; and
12.9% vs. 5.3%, P=0.02, respectively). Prediabetic subjects with SCH were
characterized by significantly higher HOMA-IR, TSH levels, UACR, and prevalence
of microalbuminuria than those with euthyroidism. TSH level was associated with
total cholesterol (P=0.05), fasting insulin (P=0.01), HOMA-IR (P=0.01), and UACR
(P=0.005). UACR was associated with waist circumference (P=0.01), fasting
insulin (P=0.05), and HOMA-IR (P=0.02). With multiple logistic regression
analysis, SCH was associated with microalbuminuria independent of confounding
variables (β=2.59; P=0.01).
 
 CONCLUSION:
 Our findings suggest that predictable subjects with SCH demonstrate
higher prevalence of microalbuminuria than their non-SCH counterparts. SCH is
also independently associated with microalbuminuria in prediabetic subjects.
Screening and treatment for SCH may be warranted in those patients."

EASY WAY  TO FIND IF YOUR AUTOIMMUNE DISEASE IS TH-1 OR TH-2
 
Hashimoto Thyroiditis is the most common type of Auto immune disease in United

A person may have more than one autoimmune disorder at the same time.
 
Finding if it is TH-1type or TH 2 type autoimmune disease  can help in the
treatment. Of course blood tests are one way of finding which type of auto immune

However there are other simpler ways to find out if your autoimmune disease is
TH-1 type or it is TH- 2 type. This test involves how you feel after taking certain
foods and supplements .
 
If after drinking coffee you feel better, and your symptoms improve then you are most
likely TH1 dominant and if your symptoms worsens then you are TH 2 dominance.
 
Similarly, green tea, grape seed extract, pine bark extract and white willow bark will
behave like coffee.
 
However mushrooms, licorice, lemon balm, echinacea, astragalus
behave opposite to the coffee. If the patient feels better and symptoms improve
then he/she is TH-2 dominance and if symptoms get worse then patient is TH -1
dominance. These patterns along with lab test can give clinicians to formulate
treatment.

HOW TO TAKE CARE OF YOUR THYROID- THE MASTER GLAND

The Thyroid is a very sensitive gland and so are the medications you take for
Hypothyroidism. These mediations can react with certain minerals,
supplements and foods. It's advisable you take Thyroid medications  first
thing in the morning, with empty stomach, and do not eat for at least
one hour.
 
There are certain foods, minerals  and medications which can interfere
with thyroid medications and I recommend to avoid them if you have
hypothyroidism. If  you have to take them, do so in modest amounts in the evenings or
at night.These foods, minerals ,medications are:
- Soya and soya beans
- Walnuts
- Food with Cotton seed
- Calcium
- Iron (including iron containing multi vitamin)
- Aluminum and /or Magnesium
- containing  medications  like antacids.

What are Goitrogenis ?
There  are  substances mostly in food variety in nature and they  can

as a  preventive measure, especially if there is a  family history of thyroid
diseases or there is incidences of thyroid disorders more
then other communities..

Iodine  is the  main food for thyroid and  is  an important part of its normal structure.
Iodine has ant proliferative and antineoplastic property. Iodine deficiency
gives the swelling of thyroid known as  a goiter. Goitrogenic foods also
interfere with enzyme process needed  for formation  of T4, T3 and for
conversion of T 4 to T 3.
 
Role of gluten free and Paleo diets for Thyroid patients
I  consider wheat  also cause  hypothyroidism byformation of anti bodied
(Hashimoto's disease) This antibodies formation occur especially if a
person has genetic predisposition. In my clinical practice I have seen some
thyroid patients especially  with Hashimoto's  feel much better after starting
gluten free diet even when their tests are negative for celiac diseases.
Some do better on paleo diet.

IMPORTANT HOME TEST 

What is an  easy, low cost  and  yet  very useful  test you can do  at home if you suspect that you might be having low  thyroid
 (Hypothyroidism) ?
 
Answer: Fasting Temperature.
Fasting temperature is very  easy, low cost, yet very important test you an do at home on your own  when you suspect you might be having low thyroid.  In Hypothyroidism morning fasting temperature may be low even if your THS is reported normal, and that will give you  clue to have further evaluation.
 
Fasting temperature should be taken first thing thing in the  morning  as soon you open your eyes, and you are still 1n bed. Use same thermometer and same location (oral or auxiliary etc.)  every day.

HOLISTIC APPROACH TO THYROID DYSFUNCTION
 
 I am  not alone, some  other respected physicians  like  Dr. David Brownstein MD  (http://w3.newsmax.com/newsletters/brownstein/thyroid_video/video.cfm) ,and Dr. Mark Hymen( below ) share the views  and concerns   that  presently many patients with Hypothyroidism  and Hashimoto's thyroiditis  are  remained   undiagnosed.  If you feel cold and tired all the time, there’s a good chance your thyroid is to blame, because one out of five women and one out of ten men have thyroid problems. There are many causes of low thyroid function, but the most common one is environmental toxins.
 
For example, plastics, pesticides, thallates in plastic bottles, BPA (bis-phenol A) in cans, parabens in sunblock and make-up, chemicals in our food and water: all of these things interfere with our thyroid function, which acts like the yellow canary in the coalmine that died when the air went bad. When our environment becomes overloaded with toxic substances, the thyroid is the first to go down.
 
What you are eating can also mess up your thyroid. Gluten is one of the biggest causes of low thyroid function, because it causes an autoimmune reaction against the thyroid. We call this Hashimoto’s disease. It is fixable. If you get rid of gluten, you can heal it.

HOW SYNTHROID  ( LEVOTHYROXIN ) CAN DAMAGE YOU ?

 
Synthroid and thyroxine can damage every cell and every organ of your body. How?
 If you are taking Synthroid or thyroxine, you may be still thyroid deficient and remained hypothyroid, Hypothyroidism if not corrected can cause serious damage to every cell and every organ of your body.  T4 CONVERSION PROBLEMS CAN GIVE THYROID DEFICIENCY 

Perhaps you are on thyroid medication and you are still symptomatic, but your TSH is “normal.” There can be many causes for this. The most important and most common cause is that the conversion of T4 to T3 is not occuring.
 
Remember that T4 is not functional itself, while free T3 is. It follow that patients receiving Synthroid (T4-only medication) suffer more. Free T3 is the one that acts on every cell and makes every cell of our body function properly. T4 has to be converted to free T3 for this to happen. However there are many situations where this conversion is interrupted. For example, excess hormones, especially estrogen, birth control pills, and the presence of heavy metals like mercury, lead, arsenic and fluoride, bromide etc. This interruption of the conversion to T3 is an indicator of hypothyroidism even when T4 and TSH are normal. 
 
Many physicians are taught to only look at TSH .They will tell their patients that their thyroid is normal, but in fact it is not. A thyroid deficiency needs to be detected and treated as early as possible. Hypothyroidism has been known to cause many secondary illnesses, some of them life-threatening like heart attacks and arrhythmia. Many are debilitating like dementia and mental retardation. Early detection of a thyroid deficiency, including T3 deficiency, is very crucial.

When a “normal” TSH doesn't tell the whole story:
You've seen one doctor after another, each treating some aspect of your suffering but none of them getting to the root of your many and varied symptoms. Finally you've figured it out: it’s the thyroid! So you get yourself to a well-respected doctor who understands these things. You’re tested for TSH, but lo and behold, it comes back “normal.”
 
What is really going on?
 
While the TSH may be normal, your thyroid antibodies may be abnormal, and perhaps you weren’t tested for this. If your thyroid disorder is given proper treatment at this stage, that treatment may reverse and prevent damage to the thyroid.
 
Thyroid dysfunctions may present many ways, and that is what can be so confusing, to patients and to their doctors.
 
Thyroid problems have been linked to more then 59 diseases like cancer, diabetes, obesity, ADHD, skin, nail and hair conditions, high cholesterol, high or low blood pressure, angina, heart attacks, strokes, allergies, carpal tunnel, fibromyalgia, chronic fatigue, arthritis and many others conditions.
 
According to Dr. David Brownstien MD, 52 million Americans are suffering from thyroid dysfunction and most of them do not even know it. He states that sixty to eighty percent of Americans may have unrecognized thyroid problems. Like oxygen, every cell in the body needs thyroid hormone.  Thyroid dysfunction will affect every cell, every organ of the body. A thyroid imbalance can cause an imbalance in other hormones as well.
 
Interestingly, the usual thyroid lab tests may be negative and may therefore be misleading. The test most commonly ordered is for TSH and it may be normal. As a result, patients are told they do not have thyroid disease. However, the doctor who takes a thorough history, performs a thorough examination and obtains fasting temperatures has the opportunity to discover when patients are still showing evidence of hypothyroidism and/or high Free T3 despite “normal” TSH levels. According to Dr. Brownstien, ever since doctors started relying on TSH alone as an indicator of thyroid disease, many cases of hypothyroidism have been missed. Consequently, not only are cancer, diabetes, heart disease and obesity on the rise but also new mysterious diseases like chronic fatigue syndrome and fibromyalgia.
 
It is important to recognize all the signs and symptoms of hypothyroidism. Some of the symptoms patients may experience are:

If you suspect you may have a thyroid issue, take your fasting temperature daily, first thing in the morning. If your temperature is below 98.2 F, hypothyroidism should be considered and further evaluation is needed.  You should also check your pulse and blood pressure. High or low pulse and blood pressure is anther indication of thyroid disease.
